Completed Twentyfour War Patrols, she was lost on her Twentyfifth patrol.

16th September 1940.
Departed the Clyde area for Fort Blockhouse, Gosport..
20th September - Arrived at Fort Blockhouse.
21st September - Whilst submerged at Fort Blockhouse she sustains damage to her foreplanes during an air raid.

First Patrol - 25th September 1940.
Departed Portsmouth to patrol in the English Channel.
5th October - Ended patrol at Fort Blockhouse.

Second Patrol - 28th October 1940.
Departed Portsmouth for Gibraltar. En-route she was to make a short 48-hour patrol in the Bay of Biscay.
6th November - West of Gibraltar Utmost whilst submerged sighted the British destroyer Encounter. She fired a smoke candle to indicate her position and expected Encounter to stop and tell her to surface which was the normal procedure. Not receiving the order to surface she fired two more smoke candles and surfaced colliding with Encounter. Both Utmost and encounter were damaged.
7th November - Arrived Gibraltar where repairs were undertaken to make her seaworthy again. Permanent repairs were to be undertaken at Malta.
30th November - Departed Gibraltar for Malta.
8th December - Arrived Malta.

Third Patrol - 4th February 1941.
Departed Malta to patrol the Western approaches to Tripoli, Libya.
9th February - Attacked an enemy convoy about North-East of Zuwara, Libya. Three torpedoes were fired but no hits were obtained.
12th February - Torpedoed and damaged the Italian merchant ship Mauly off Zuwara, Libya.
15th February - Ended patrol at Malta.

Fourth Patrol - 25th February 1941.
Departed Malta to carry out a special operation. Upon completion of the special operation she patrolled off the Gulf of Hammamet.
4th March - Ended patrol at Malta.

Fifth Patrol - 6th March 1941.
Departed Malta to patrol off the Gulf of Hammamet.
9th March - Torpedoed and sank the Italian merchant Capo Vita off the Gulf of Hammamet North-East of Sousse, Tunisia.
12th March - Ended patrol at Malta.

Sixth Patrol - 19th March 1941.
Departed Malta to patrol off Kerkennah.
28th March - Attacked a convoy of five German merchant ships and three Italian destroyers and torpedoed and sank the German merchant ship Heraklea and torpedoed and damaged the Ruhr south-east of Kuriat, Tunisia.
1st April - Ended patrol at Malta.

Seventh Patrol - 17th April 1941.
Departed Malta to carry out a special operation.
22nd April - Ended patrol at Malta.

Eighth Patrol - 25th April 1941.
Departed Malta to patrol in the Gulf of Hammamet.
9th May - Ended patrol in Malta.

Ninth Patrol - 24th May 1941.
Departed Malta to patrol off the Gulf of Hammamet and to carry out a special operation.
29th May - Attacked a merchant vessel with three torpedoes about South-East of Kuriat. No hits were obtained.
4th June - Ended patrol at Malta.

Tenth Patrol - 17th June 1941.
Departed Malta to patrol in the Tyrrhenian Sea to the North of the Straits of Messina.
24th June - Llanded a sabotage party North of Amantea, Italy that succesfully destroyed a railway.
26th June - Torpedoed and sank the Italian merchant ship Enrico Costa near Cape Todaro, Sicily, Italy.
3rd July - Ended patrol at Malta.

Eleventh Patrol - 17th July 1941.
Departed Malta to patrol in the Tyrrhenian Sea to the North of the Straits of Messina. She also was to destroy a railway if the opportunity arose.
27th July - Landed a sabotage party near Pizzo, Italy. They succesfully destroyed a train.
28th July - Torpedoed and sank the Italian merchant ship Frederico in the southern Tyrrhenian Sea off the west coast of Calabria, Italy.
34d August - Ended patrol at Malta.

Twelfth Patrol - 19th August 1941.
Departed Malta to patrol off South coast of Calabria, Italy. Once again she carried a raiding party to destroy a railway if the opportunity arose.
27th August - Landed a sabotage party near the mouth of the river Seracino, Italy. They succesfully destroyed a railway bridge.
28th August - Attacked an enemy merchant vessel near Punta Alice, Italy. Three torpedoes were fired but no hits were obtained.
31st August - Ended patrol at Malta.

Thirteenth Patrol - 12th September 1941.
Departed Malta she was ordered to proceed to a position near the Kerkennah Shallows, to pick up the crew from a crashed Blenheim bomber.
13th September - Sighted distress. Sighted a rubber dinghy with three occupants. They were the missing aircrew and were quickly picked up.
14th September - Ended patrol at Malta.

Fourteenth Patrol - 18th September 1941.
Departed Malta to patrol in the Southern Tyrrhenean Sea. Once again she carried a raiding party to destroy a railway if the opportunity arose.
22nd September - Landed a sabotage party near Intavolata, Italy. They were not succesful in destroying a railway tunnel as they were detected by Italian guards.
23rd September - Landed a sabotage party near Intavolata, Italy. They were not succesful in destroying a railway bridge over the river Oliva as, yet again, they were detected by Italian guards.
2nd October - Attacked an enemy convoy off Martettimo Island. Only one torpedo was fired instead of the intended three as she was detected by the escorts and the attack had to be broken off.
3rd October - Ended patrol at Malta.

Fifteenth Patrol - 23rd October 1941.
Departed Malta to patrol off Kuriat and also to perform a special operation.
26th October - Special operation completed.
1st November - Destroyed the already grounded and damaged Italian merchant ship Marigola off Kuriat Island, Tunisia.
3rd November - Ended patrol at Malta.

Sixteenth Patrol - 15th November 1941.
Departed Malta to patrol off the West coast of Greece. This was later changed to the Southern approaches to the Strait of Messina.
21st November - Torpedoed and damaged the Italian heavy cruiser Trieste South of the Straits of Messina.
27th November - Ended patrol at Malta.

Seventeenth Patrol - 9th December 1941.
Departed Malta to patrol off the Gulf of Taranto.
12th December - Attacked the Italian transport Fabio Filzi in the Gulf of Taranto. Both torpedoes fired missed their target.
16th December - Attacked a force of Italian warships off the Gulf of Taranto. Four torpedoes were fired from long range but no hits were obtained.
19th December - Ended patrol at Malta.

3rd January 1942.
Departed Malta for UK via Gibraltar.
12th January - Arrived Gibraltar.
17th january - Departed Gibraltar for Holy Loch.

Eighteenth Patrol - 25th January 1942.
Whilst on passage from Gibraltar for Holy Loch ordered to take up a patrol position South-West of Brest, France.
2nd February - Left her patrol area and resumed her passage to Holy Loch.
5th February - Ended patrol at Holy Loch.

8th February 1942.
Departed Holy Loch for Blyth.
11th February - Arrived Blyth.
13th February - Refitting at Blyth.
23rd April - Refit completed.

3rd May 1942.
Departed Blyth for Holy Loch.
6th May - Arrived Holy Loch.

On completion of work-up and exercises.

8th June 1942.
Departed Holy Loch for Malta via Gibraltar to rejoin the 10th Submarine Flotilla.

Nineteenth Patrol - 3rd July 1942.
Departed Gibraltar for a work-up patrol in the Alboran Sea.
10th July - Ended patrol at Gibraltar.

Twentieth Patrol - 1st August 1942.
Departed Gibraltar for Malta and to patrol in the Sicilian narrows during 'Operation Pedestal'.
10th August - Fired three torpedoes against the Italian merchant ship Siculo off Marettimo. All torpedoes missed their target.
15th August - Ended patrol at Malta.

Twentyfirst Patrol - 25th August 1942.
Departed Malta to patrol off the West coast of Greece.
2nd September - Attacked the Italian merchant ship Nerucci with three torpedoes West of Levkas Island, Greece. No hits were obtainedd.
7th September - Ended patrol at Malta.

Twentysecond Patrol - 17th September 1942.
Departed Malta to patrol in the Southern Tyrrhenean Sea.
18th September - Ordered to patrol off Empedocle, Sicily to find out if there was any E-boat activity originating from that port.
22nd September - Ordered to proceed to the Gulf of Hammamet.
25th September - Carried out a bombardment of Linosa Island.
26th September - Ended patrol at Malta.

Twentythird Patrol - 3rd October 1942.
Departed Malta to patrol in the Tyrrhenian Sea. She was also to carry out a special operation.
9th October - Carried out special operation 'Blackbird'. Two agents were to be landed near Napoli. It appeared the agents were detected by the enemy during the landing.
11th October - Attacked but missed a small merchant vessel with two torpedoes South-West of Civita Vecchia, Italy.
13th October - Torpedoed and sank the German tanker Languste (off Cape Figari, Sardinia.
17th October - Ordered to take up a patrol position between Pantelleria and Lampedusa to intercept an enemy convoy to Tripoli, Libya.
19th October - Attacked an enemy convoy between Pantelleria and Lampedusa. Two torpedoes were fired but no hits were obtained.
21st October - Ended patrol at Malta.

Twentyfourth Patrol - 3rd November 1942.
Departed Malta to patrol off 'The toe of Italy'.
7th November - Attacked an Italian submarine with four torpedoes near Capo dell Armi. No hits were obtained.
10th November - Attacked an enemy cruiser force with four torpedoes East of Augusta, Sicily, Italy. No hits were obtained.
12th November - Ended patrol at Malta.

Twentyfifth Patrol - 17th November 1942.
Departed Malta to patrol in the Tyrrhenean Sea.
